Blake’s House of Independence (BHI) provides a comprehensive range of home and community-based services designed to promote health, safety, independence, and quality of life. Services are individualized to meet each client’s assessed needs and are delivered in accordance with the client’s authorized plan of care, applicable state and federal regulations, and agency policies.
Depending on the client’s needs and level of care, services may be provided by qualified Personal Care Assistants (PCAs), Companion/Sitters, Licensed Practical Nurses (LPNs), Registered Nurses (RNs), or other authorized healthcare professionals. Each service is delivered with dignity, respect, professionalism, and a commitment to person-centered care.
